The first step in becoming a patient at Healing Hyperbarics is to complete a Registration Form. At the bottom of that form you will be asked to provide a log-in and password, which will be required for you to access the Initial Medical History Form. All forms that are submitted online will be transmitted to us as encrypted information so that this private medical information cannot be accessed by anyone except you and our office. You will also need to download and complete the Consent for Treatment and E-mail Authorization forms and fax (813-960-3465) or scan and email to info@healinghyperbarics.comTo read the Health Insurance Portability and Accountability Act (HIPAA) notice which describes how medical information about the patient may be used and disclosed and how you can obtain access to this informaiton click here.
Once an Initial Medical History Form, the Consent for Treatment and E-mail Authorization Forms have been submitted, our Medical Staff will review the patient’s medical history. You will then be notified if any additional medical information is needed. If it is recommended by the Physician to proceed with Hyperbaric Therapy, you and the Physician can decide which Service Option (In-home or In Our Office) will be utilized, as well as the therapy schedule.
Once therapy has started, you will be required to complete a Follow-Up Medical History Form every 15 days for the first 60 days of therapy. This form will be accessable after you log-in to the account. You will be notified by e-mail the day before the form must be completed, and you will be sent a reminder e-mail after 2 days if the Follow-Up Form is not completed by the scheduled date. You are also responsible for submitting occasional Follow-Up Forms if long-term Hyperbaric Oxygen Therapy is to continue. You may submit additional Follow-Up Forms as often as you wish if there is additional/new information that you feel should be added to the medical record.
